CASPer for CRNA and Anesthesia Programs: 2026–27 List
A growing number of US anesthesia programs require CASPer. For 2026–27, Acuity lists seven nurse anesthesia (CRNA) programs, including Wake Forest, Marquette and Hofstra-Northwell, and four anesthesiologist assistant programs, including Case Western and Nova Southeastern. All of them use United States Casper 2, so one sitting can be sent to every program on this list.
Nurse anesthesia (CRNA) programs that require CASPer
These programs appear under Graduate Nursing on Acuity's Dates and Fees page for 2026–27:
- Franciscan Missionaries of Our Lady University: Nurse Anesthesia
- Hofstra-Northwell School of Graduate Nursing: Nurse Anesthesiology
- Marquette University: Nurse Anesthesia
- Mary Baldwin University: Nurse Anesthesiology
- Millikin University and Decatur Memorial Hospital: Nurse Anesthesia
- Union University: Nurse Anesthesia
- Wake Forest University School of Medicine: Nurse Anesthesia
The same Graduate Nursing list also includes non-anesthesia programs, such as Hofstra-Northwell's nurse practitioner program, Morehouse School of Medicine nursing and several Texas Woman's University streams. If you are applying to a nurse practitioner program, check that list too.
Anesthesiologist assistant programs that require CASPer
Acuity lists these under Anesthesia for 2026–27:
- Case Western Reserve University: Anesthesiologist Assistant
- Nova Southeastern University: Master of Science in Anesthesia
- University of Colorado: Anesthesiologist Assistant
- University of New Mexico: Master of Science in Anesthesia
Anesthesiologist assistant programs are a different route from CRNA programs, usually for applicants with a science background rather than ICU nursing experience. If you are applying to both types, you can still use one Casper 2 sitting, because they share a test type.
Which CASPer test to book
Acuity's test-type guide says applicants to US graduate business or professional health sciences programs select United States Casper 2. Every program above is listed under that test type (CSP-20100). The same test is used by US physician assistant, physical therapy, dental and veterinary programs, so a single sitting can also go to those.
A CASPer score is valid for one admissions cycle and one test type, and you can take each test type only once per cycle. Pick a date before the earliest program deadline, and add every program to your distribution list. See how to send CASPer scores.
How CASPer fits a CRNA application
CASPer is an additional requirement, not a replacement for clinical prerequisites. Wake Forest's CRNA entrance requirements, for example, list a BSN, a minimum 3.0 GPA in nursing and supporting courses, a satisfactory GRE score within the past five years, completion of the Casper assessment and a CCRN score report, alongside at least one year of ICU experience.
Programs use CASPer to see how applicants handle interpersonal and ethical situations, which matters in a role built on teamwork and patient safety. How much weight it carries varies by program; see how important CASPer is.
Preparing for CASPer as an ICU nurse
Your bedside experience is an asset, but CASPer scenarios are everyday situations, not clinical cases. A few points specific to experienced nurses:
- Don't answer like a protocol. Raters want your reasoning, including the perspectives of the people involved, not a policy recital. Our answer structure guide shows the balance.
- Use real experience in reflection prompts. ICU work gives strong examples of teamwork, conflict and resilience. Try the self-awareness and resilience workbooks.
- Practise the clock. The test gives 1 minute per video question and 3.5 minutes per pair of typed questions, per Acuity's format guide. Shift work makes short, regular practice sessions easier than long ones.
- Watch for professionalism traps. Scenarios about colleagues cutting corners are common. See professionalism questions.
Frequently Asked Questions
Do CRNA schools require CASPer?+
Some do. For 2026–27, Acuity lists nurse anesthesia programs at Franciscan Missionaries of Our Lady, Hofstra-Northwell, Marquette, Mary Baldwin, Millikin and Decatur Memorial Hospital, Union University and Wake Forest.
Which CASPer test do CRNA programs use?+
United States Casper 2, listed as CSP-20100 on Acuity's 2026–27 Dates and Fees page. The anesthesiologist assistant programs on Acuity's list use the same test type.
Does CASPer replace the GRE for CRNA programs?+
No. It is an added requirement. Wake Forest, for example, lists both a GRE score and completion of the Casper assessment, alongside a CCRN score report and ICU experience.
Do I need clinical knowledge for CASPer?+
No. CASPer uses everyday scenarios and assesses judgment and communication. Clinical experience helps you find good examples for reflection prompts, but it is not tested directly.
